Vezikulobulózne lézie predstavujú zriedkavú kožnú manifestáciu kožného a/alebo systémového lupus erytematosus (SLE) s heterogénnou klinickou manifestáciou. Kým miernejšie formy môžu viesť ku kozmetickým problémom a diskomfortu pacienta, závažné varianty predstavujú potenciálne život ohrozujúce stavy. Bulózny systémový lupus erytematosus (BSLE) predstavuje zriedkavú kožnú manifestáciu SLE, charakterizovanú tvorbou autoprotilátok proti kolagénu typu VII. Autori prezentujú kazuistiku mladej pacientky so známym SLE, u ktorej sa rozvinul generalizovaný bulózny exantém. V priebehu liečby pacientka absolvovala terapiu takmer všetkými dostupnými liečivami, avšak s nedostatočným terapeutickým efektom alebo s rozvojom závažných alergických reakcií. Dobrú klinickú odpoveď dosiahla až po terapii rituximabom v kombinácii s metotrexátom. Článok prináša prehľad možností liečby tohto zriedkavého ochorenia.

Abstract
Vesiculobullous lesions represent an uncommon cutaneous manifestation of cutaneous and/or systemic lupus erythematosus (SLE) with heterogeneous clinical presentation. Whereas milder forms may lead to cosmetic impairment and patient discomfort, severe variants can manifest as potentially life-threatening states. Bullous systemic lupus erythematosus (BSLE) is a rare cutaneous manifestation of systemic lupus erythematosus (SLE), characterized by the production of autoantibodies against collagen type VII. The authors present a case report of a young female patient with established SLE who developed a generalized bullous eruption. During the course of treatment, the patient received almost all available therapeutic options; however, the response was insufficient or treatment was complicated by the development of severe allergic reactions. Finally, a good clinical response was achieved after rituximab treatment in combination with methotrexate. This article reviews current therapeutic options for this rare condition.
